Well, I didn't either, until about two weeks ago. That's when In-Form™ was completed. It lets you create your form with a control panel. The JavaScript can be pasted into any web page on the Internet, as many web pages as you want. There is nothing else to install. When you want to change the form, maybe the design or the destination email address or adding text/HTML radio buttons, simply use the control panel again.
