Simple Feedback Form
You'll see a simple feedback form. If you fill it out, the next page you'll see is the "thank you" page, which in this case is the same page you're reading right now. Behind the scenes, an email is sent to us, which could be sent to you (or anyone else you designate) if Master Form V4 was installed on your server.
Simple Feedback Form With Autoresponse To Form User
You'll see the same simple feedback form as above. The difference is in how the form submission is handled. As before, if you fill it out, the next page you'll see is the "thank you" page, which in this case is the same page you're reading right now. This time, however, in addition to the email sent to the webmaster, an email is also sent to the email address the form user provided — you.
Busy Feedback Form
This feedback form updates a CSV file with the information submitted, sends an email to the webmaster with the information (which could be sent to you, or anyone else you designate, if Master Form V4 was installed on your server), and gives the form user an opportunity to subscribe to an ezine. A custom error page is used if the form user should make an error like an incorrectly formatted email address or omitting required information. The "thank you" page is personalized.
Subscribe/Unsubscribe Form
This can take the place of the old Master Subscriber Lite and Master Subscriber Pro. The form asks for name and email address, and has radio buttons to choose whether subscribing or unsubscribing.
Multiple Ezine Subscription Form
This can take the place of the old Master Subscriber Pro. The form asks for name and email address, and has three checkboxes for the user to choose which ezine(s) to subscribe to.
Multi-page Form
This is a 2-page form. Once both pages are filled out, the next page is a printable page containing the information provided on the form. Behind the scenes, Master Form V4 sends an email to the webmaster and updates a CSV database file with the same information.
File Upload Form
Here is a form that can be used to upload up to two files. The files are to be stored on the server and also attached to email sent to the webmaster. The email will contain the other information provided on the form, and the form information will also be stored in a tab-delimited database on the server. If you use the form, the page you'll see after the files are uploaded is the "thank you" page, which in this case is the same page you're reading right now.
3-Page (complete with contact page/form) Site Replication Form
This form creates a 3-page web site — home page, article page, and contact page. (It's a working contact form.)
